The Maryland Medical Order for Life-Sustaining Treatment (MOLST) is a standardized form containing an individual's personal medical choices for life-sustaining treatment. It can be viewed as a “snapshot” of an individual’s Advance Directive and increases the likelihood that her or his treatment wishes are honored throughout the healthcare system. The form may be revised at any time, however is only considered valid when signed by a physician or nurse practitioner.
